What happens during a new patient appointment?
At your first appointment, we intend for you to meet your dental team, have time to express any concerns you may have, and ask questions. Diagnostic records will be taken, which may include X-rays (if your previous dentist has X-rays that are less than two years old, please have them forwarded to our office before your exam).
Following a comprehensive examination, your dentist will discuss their findings and explain any treatment recommendations. A professional cleaning may also be completed at this visit, depending on the condition of your periodontal health.
New and Returning Patient Forms
To help make your visit as smooth and efficient as possible, we offer convenient online forms for both new and existing patients. Completing your paperwork ahead of time allows our team to prepare for your appointment, review important health information, and minimize wait times when you arrive. If you have any questions while filling out your forms, our office is always happy to assist you.

How should I prepare for my first visit at Cotswold Family Dentistry?
Getting ready for your first visit? Here’s what to bring and know before you arrive:
- Complete your new patient forms before your appointment, and be sure to include your insurance card information (if applicable) to help streamline your check-in process.
- Bring a valid ID and your insurance card, even if you already entered the information on your new patient forms, so we can verify your coverage if needed.
- Have a list of any current medications you’re taking.
- Arrive a few minutes early to relax and enjoy our complimentary beverage bar.
- Be prepared to discuss your dental history, concerns, and smile goals so we can personalize your care.
Note: Insurance verification can take up to 48 hours. Providing your insurance information as early as possible helps ensure your benefits can be verified before your appointment.
